A Different Kind of Care — All in One Place

PACE, the Program of All-Inclusive Care for the Elderly, is a Medicare and Medicaid program that helps older adults get the care they need in their homes and community. As a PACE participant, you must receive all healthcare services, including primary and specialty care (other than emergency services), from PACE Georgia or an authorized entity. You may be personally and fully responsible for unauthorized or non-PACE services.

PACE Georgia is designed for individuals who are: 

• Are age 55 or older

• Live in one of the service area zip codes:

  • Chatham County: 31302, 31322, 31328, 31401, 31404, 31405, 31406, 31407, 31408, 31409, 31410, 31411, 31415, 31419, 31421

  • Effingham County: 31308, 31318, 31324 

  • Bryan County: 31324

• Have been assessed by your state as needing nursing facility level of care

• Are able to live safely at home with services provided by PACE
  

PACE Georgia is typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. Participants who do not qualify for Medicaid will be charged a monthly fee and have the option to privately pay.

PACE provides you and your family with multiple layers of support that enable you to live at home, where you want to be. With services designed around your needs and personal goals, you can live your life the way you want to, with us by your side.

Once enrolled in PACE, there are no deductibles, copays, or benefit limits for any prescription medications, medical care, or services approved by your PACE care team. PACE Georgia is typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. Participants who do not qualify for Medicaid will be charged a monthly fee and have the option to privately pay.

You will meet with your dedicated care team to develop a personalized care plan that promotes your happiness and well-being. Our drivers will take you to doctor’s appointments, our PACE center will be available for activities and meals, and you’ll have a dedicated partner in your health journey.

PACE services include all your medical care, vision, dental, social services, rides to doctors and the PACE center, nutritional services, help at home, fun activities, and so much more. Learn more about the services PACE Georgia provides here.

If your PACE Georgia team cannot directly provide a specific medical service, we will authorize services to see a specialist. We will schedule the appointment, provide rides to your appointment, and follow up with the enrollment specialist to ensure you get the care you need.

PACE is designed to support you so that you can live independently in your home. Our PACE centers are open Monday through Friday during normal business hours, so participants head home at the end of the day. For people who need additional care, we can provide help at home.

If PACE isn’t for you, you may voluntarily disenroll. Those with Medicare or Medicaid will be assisted in returning to their former healthcare coverage. A participant will never be disenrolled due to changes in health status.

When you enroll in PACE Georgia, you agree to receive all your healthcare services through our program. We have our own team of providers and contract with a network of healthcare professionals to ensure all your medical needs are met. PACE Georgia replaces your insurance, coordinating all your healthcare services, managing appointments, and handling claims on your behalf. While PACE Georgia guarantees access to all necessary services, we do not guarantee access to a specific provider. Our care team will ensure you receive the right services through our PACE center and provider network, delivering high-quality, personalized care.

At PACE Georgia , your satisfaction and well-being are our top priorities. If you are ever unhappy with the care or services, you receive—or if you disagree with a decision we’ve made regarding your care—you have the right to file a grievance or appeal at any time. Grievances may be submitted verbally or in writing and do not require the use of the grievance form.  The process is outlined in your Enrollment Agreement, and our team is always here to help. Please speak with any staff member or call your local PACE Georgia center—we will guide you every step of the way.

If you would like someone to file a grievance or appeal on your behalf, you may appoint a representative. This could be a family member, friend, caregiver, or someone you trust. Once completed, please return the form to your local PACE Georgia center or give it to your care team.
